Nose Aesthetics - Rhinoplasty
Op. Dr. Bahar Kayahan Sirkeci
Eyes are the most beautiful and striking part of the face. The aim of rhinoplasty (nose aesthetics) is to emphasize the eyes by providing a nose appearance suitable for facial anatomy. However, a successful rhinoplasty surgery is only possible if the shape of the nose is compatible with the face and personal expectations, as well as preserving and improving breathing, smell and the functions of sinuses.
Every face is different, in this case, the anatomy of the nose varies from person to person and therefore rhinoplasty is one of the most difficult and complex surgeries. The fact that the anatomy of the nose and face is unique to the individual requires the use of personalized techniques. There is no standard face, standard nose and standard rhinoplasty.
In preoperative interviews, the patient’s facial features should be carefully examined, the patient should be carefully examined in terms of the width / length differences between the right and left face, the extension of the nose on the face, the distance between the eyes and the height difference, if any, the distance between the nose-lip, the shape, angular and positional differences of the nostrils, and the patient should be informed about the best possible result.
The patient should be fully examined before the surgery and any intranasal curvatures, stenosis and problems of the sinuses opening into the nose should be revealed. This will provide both the correct planning of the surgery and clearer information about the postoperative healing process.
There are two types of techniques in rhinoplasty: closed (without external incision) and open (with an incision at the tip of the nose near the lips). The type of surgery will be performed is planned in the preoperative patient interview according to the surgeon’s experience, the shape and functional problems of the nose, primary (first nose surgery) or revision (second or more) rhinoplasty surgery. In patients who have previously undergone intranasal or rhinoplasty, the possible need for cartilage support from the ear or rib should be explained to the patient in detail before surgery.
The preoperative interview is very important in terms of understanding the patient’s expectations, explaining to what extent the doctor can meet these expectations (surgical skill and experience), and establishing a healthy and reliable patient-physician relationship in the postoperative process. Surgery should be postponed if the patient has any questions in his/her mind or if he/she thinks that the physician’s explanations about the surgery are not fully clear.
It may take 6-12 months for the final shape of the nose to form after the surgery. The most difficult process after surgery is the first two weeks. Since it is an important large bone structure intervention in the middle of the face, postoperative care is very important. Careful care of both the physician and the patient; such as ice applications, care creams, plaster and bandage process are important. It is very important not to wear glasses for the first month, to avoid activities that require heavy exertion, and to avoid blows to the nose and face. Masks that suppress the tip of the nose should be avoided for the first 3 months. The patient’s healing process should be carefully monitored with weekly follow-ups for the first month and monthly follow-ups afterwards.
Both the physician and the patient want to achieve a successful result with a single operation. No physician wants his/her surgery to end in an undesirable way. However, in case of undesirable results and complications that may develop due to problems that may occur during the healing process, it is important that the physician can solve these problems and provide this support to the patient.
As I say to all my patients, it is very important that you establish a healthy and reliable relationship with your physician in this surgery that will create a permanent change in your face and that you have surgery with someone who you really feel comfortable with.
